Learning from Incident training aim to demonstrate the chain of mistakes that led to an accident and the measures that need to be taken to prevent a recurrence. This training has been prepared to demonstrate a real-life incident onboard an oil tanker that concluded with a serious injury after a hot water burn during the main engine maintenance.
